Job Title: Registered Nurse Job Specialty: Home-Hospice Care Operations Job Duration: 13 Weeks Shift: 8hr Day Shift (40 hours per week) Guaranteed Hours: Not specified Experience: Minimum 1 year Home Health RN experience License: Active RN license (IL) Certifications: Active BLS (AHA) Must-Have: - Wound care experience, including wound vacs, pleural drains, and ostomy care - Proficiency in completing peripheral blood draws, central line care, and IV administration through multiple devices (elastomeric, IV pumps) - Excellent assessment skills - Experience with computer documentation - Reliable transportation, active driver's license, and insurance Job Description: - This position operates 100% in the field, requiring the nurse to drive from one patient to the next. - There is no physical office to report to; all meetings are held virtually, and nursing supplies are delivered. - Initial on-site training is required for 2-3 days. - The Case Manager can expect to see 20-30 patients per week, with an expectation of 6 routine visits per day. - Patients typically have multiple comorbidities, which may include CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical care, wound care, ostomy care, labs, IV infusion, etc. - Responsibilities include educating patients on medications, disease management, self-care, and performing nursing skills such as wound care, infusion, line maintenance, and ostomy care. - Educate and train patients on performing care for their conditions. - Use EPIC for computer charting and Sapphire IV pumps. - A laptop for charting will be provided. - Any solid color scrubs are acceptable for this role. - Coverage area includes Algonquin, Carpentersville, Sleepy Hollow, and surrounding towns. Interviews: The candidate will undergo a first-round screening interview with the clinical team, followed by a final interview with the manager.
