TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Oak Brook, Illinois, 60523 Pay Information $3,252 per week About The Position Job Title: Registered Nurse Job Specialty: Home-Hospice Care Operations Job Duration: 13 weeks Shift: 8hr Day Shift (40 hours per week) Guaranteed Hours: N/A Experience: Minimum of 1 year Home Health RN experience License: Active RN license in IL Certifications: Active BLS (AHA) Must-Have: - Wound care experience, including wound vacs, pleural drains, and ostomy care - Ability to perform peripheral blood draws, central line care, and IV administration through multiple devices (elastomeric, IV pumps) - Excellent assessment skills - Experience with computer documentation - Reliable transportation, active driver's license, and insurance Job Description: - This position is 100% field-based, requiring driving from one patient location to another. - All meetings are held virtually, and nursing supplies are delivered to your location. - Initial training is conducted on-site for 2-3 days. - As a Case Manager, you can expect to see 20-30 patients per week with an expectation of 6 routine visits per day. - Patients typically present with multiple comorbidities, including but not limited to CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall/fracture, infections, cellulitis, post-surgical needs, wound care, ostomy care, labs, and IV infusion. - Care responsibilities include educating patients on medications, disease management, and self-care, along with performing wound care, infusion, line maintenance, ostomy care, etc., and instructing patients on how to perform these care routines themselves. - Utilize EPIC/Computer charting and Sapphire IV pumps. - A laptop will be provided for charting purposes. - Any solid color scrubs are acceptable. - You must provide your own transportation for all patient visits within the designated coverage area. 26710814EXPTEMP Job Requirements Required for Onboarding BLS Pharmacology Skills Checklist Specialty Exam 5 Benefits of Travel Nursing A diverse portfolio of experiences adds depth to your professional profile.
