Job Description:

The Peer Support Specialist (PSS) for the Green Door Center will provide crisis stabilization services and mobile crisis services in the community and facilitate follow-up services to secure safe disposition for clients of all ages in immediate crisis. The PSS must demonstrate knowledge of identification of needs, symptoms, and behaviors of individuals who FHC serves. The PSS will work within a family's home, other community agencies, and will transport clients when appropriate.Essential Duties and Responsibilities

  • The essential functions may include, but are not limited to the following:
  • Provide crisis services to adults, children and families within the community.
  • Provide collaboration and advocacy for positive outcomes for adults, children and families in a crisis situation.
  • Develop and maintain positive relationships with both internal and external customers.
  • Remain current with all required training and certifications.
  • Perform triage of telephone contacts and on-site crisis situations to distinguish between routine, urgent, and emergent situations, complaints, and difficulties.
  • Provide crisis support and intervention either by phone or in person to individuals in psychiatric crisis.
  • Facilitate safe and adequate dispositions as dictated by the nature of the individual's difficulties.
  • Identify appropriate family and community resources and gather appropriate release documents to arrive at disposition of client in crisis.
  • Perform all necessary clinical documentation in a timeframe that is consistent with the expectation as documented in the Family Health Center policies and procedures.
  • Consult with other disciplines, Family Health Center departments, colleagues, administrators, law enforcement, first responders, etc. in an effort to communicate, coordinate, and deliver quality, effective services to individuals in crisis.
  • Provide transportation for clients in crisis when appropriate to ensure safe disposition or attendance at follow-up services.
  • Under the direction of the Therapist Coordinator, the Peer Support Specialist will provide crisis services to those in the community and at the Green Door Center.
  • Other duties as assigned.Performance RequirementsPhysical Demands and Work EnvironmentThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. Reasonable accommodation may be made to enable individuals with disabilities to perform the functions.Mental: Good judgment is required. Ability to communicate clearly both verbally and in writing with the treatment team as well as outside agencies. Must be able to engage in cooperative relationships with both co-workers and community partners.Physical: Must pass Family Health Center's physical exam. Occasional travel required for training and continued education.Working Conditions: Clean, well-lit, heated, and ventilated office. Will also work in the family's home, other community agencies, and transport clients by car and/or van.License/Certification: Driver's License (Required), must be able to attain certification as a DMHA Peer Support Professional and DMHA Crisis Peer Support Specialist.Minimum Qualifications (Knowledge, Skills, and Abilities)Experience:
    • Must be at least 23 years-of-age with at least one year of recovery, three years preferred.
    • Must have a history of lived experience with substance use or mental illness, and/or experience with a family member with substance use or mental illness history.
    • High School Diploma or G.E.D. required.
    • Sensitive and flexible when dealing with adults, adolescents, and children. Recovery Specialist must demonstrate knowledge of identification of needs, symptoms, and behaviors of individuals served.
    • Must be certified by Division of Mental Health and Addictions or willing to become certified within 6 months of employment start.
