TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Oak Brook, Illinois, 60523 Pay Information $3,252 per week About The Position Job Title: Registered Nurse Job Specialty: Home-Hospice Care Operations Job Duration: 13 weeks Shift: 8-hour Day Shift, 40 hours per week Guaranteed Hours: Not specified Experience: Minimum of 1 year Home Health RN experience License: Active state RN license required Certifications: Active BLS (AHA) required Must-Have: - Wound care experience, including wound vacs, pleural drains, and ostomy care - Ability to complete peripheral blood draws, central line care, and IV administration through multiple devices (elastomeric, IV pumps) - Excellent assessment skills - Experience with computer documentation - Reliable transportation, active driver---s license, and insurance Job Description: - This position requires working 100% in the field with travel from one patient to the next. - Initial training required on site for 2-3 days. - Case Manager can expect to see 20-30 patients per week with an expectation of 6 routine visits per day. - Patients typically have multiple comorbidities such as CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, falls, fractures, infections, cellulitis, post-surgical needs, wound care, ostomy care, labs, and IV infusion. - Provide care and education on medications, disease management, and self-care. - Perform skills such as wound care, infusion, line maintenance, ostomy care, and teach patients how to perform these care tasks. - Use EPIC/Computer charting with a provided laptop for charting. - Utilize Sapphire IV pumps. - Meetings will be held virtually, and nursing supplies will be delivered to your location. - Solid color scrubs are acceptable attire. - Coverage area includes specific regions as primary and secondary locations. 27148003EXPTEMP Job Requirements Required for Onboarding BLS Pharmacology Skills Checklist Specialty Exam 5 Benefits of Travel Nursing As a travel nurse, you'll work in various healthcare settings, gaining exposure to diverse patient populations, medical procedures, and healthcare systems.
